Items Patients Need to Bring to Visit
Patients should bring Insurance cards, Photo ID, and List of current medications, all X-rays, MRI's and other scans relevant to their condition. Please bring the actual films or CD, not just the reports. Previous operative reports
and photographs are very helpful. We will provide disposable shorts and shirts, but patients may wish to wear clothing that allow the injured area of the body to be
You will be asked to complete the following forms at your initial appointment with the doctor. You may find it helpful and time saving to print them out in
advance and bring the completed forms with you to your first visit.

Nothing to eat or drink 8 hours prior to surgery, except necessary medications with a sip of water. Arrange for a ride to and from the hospital. Wear comfortable
clothes. If you already own crutches or a walker and plan on using them after surgery, bring them with you. Notify your family physician and your health plan. If you
are having joint replacement surgery, discharge plans (eg inpatient rehab or home health) can be arranged preoperatively.
You will be discharged with a sheet of post-operative instructions. Please refer to it often in the first few days. You will be discharged with prescriptions for
medications. Please make arrangements to have these picked-up or delivered. Ice bags to the operative area generally helps to decrease pain. Call for medicaton refills
during normal office hours when we have access to your chart. We do not refill prescriptions on nights and weekends.
After Hours Process
We can be reached via our answering service at any time 352-683-0007. Routine messages will be answered when the office opens, and the doctor on call can be paged
for emergencies. If you are unable to reach the doctor on call during an emergency, please go to your local emergency room and have them try to contact us.
Dr. Jason is a salt water fish and snorkeling enthusiast. He also enjoys golfing and cycling.
Odysseus ("Ody") will often make a visit to the Spring Hill office. Ody is an AKC Canine Good Citizen and a registered Therapy Dog who
helps to brighten our patients' visits.
